A very common component of mostly all the Field service applications, in which technician/ service engineer needs a checklist to verify pre-service/ post-service process compliance. A simple list of checkboxes can go a long way in determining the streamlined execution of any service request. This also aids in audits/ documentation purposes, for admin to verify all the necessary steps were followed and adhered to.

- Checkbox based form to verify all questions are checked before proceeding to the next step.
- Mandatory/ Non-mandatory questions: Mandatory questions must be checked for user to proceed, Non-mandatory can be skipped
- Select one of the group (Question Type): A group of multiple questions – Only one should be selected among the group
- Types of questions: Checkbox, One of the Group, Yes/No, Textbox, dropdown list
- Dynamic Template: UI should be dynamic to support number of questions as per the JSON input.
- Skin and look properties should be exposed to let developer configure fonts, skin color for rebranding.
- Label (Title) of the form configurable from the properties of the component.
